Deciphering CMS Home Health Quality Reports

What are the Quality Measure (QM) Reports?

The Internet Quality Improvement and Evaluation System (iQIES) QM reports are on-demand reports that provide home health agencies with Quality Reporting Program (QRP) measure data for up to a rolling 12-months of measure data results.

There is an agency-level report and a patient-level or “tally” report for both Outcome Measures and Process Measures. These reports allow agencies to track their quality measure data regardless of quarterly submission deadlines.

Assessment-based (OASIS) measure data is updated twice per month on both the agency-level reports and the tally reports. Each update contains a rolling 12-months of data. Claims-based measure data is updated annually and only on the agency-level reports. There are no QM reports with patient-level claims-based measure data.

Outcome Measure Reports

Home health agencies can use their Outcome Report to monitor their outcome measures. This agency-level Outcome Report has four comparison rates:

  • HHA Observed – the current agency performance (not risk-adjusted) for a measure for the selected current period
  • HHA Adjusted Prior – the risk-adjusted agency performance for a measure for the selected prior period
  • HHA Care Compare Risk-Adjusted Rate (if applicable) – the agency’s Care Compare risk-adjusted performance for the measure for the selected period
  • National Observed Rate – the average performance of all agencies that have a home health stay for the selected period

The Outcome Tally Report can be used by agencies to look at measure data at the patient-level. The report indicates quality episodes where measures were triggered on the Outcome Report. These reports can be used to further investigate undesirable outcomes and develop education and/or new policies and procedures to avoid recurrence in the future.

Process Measures Reports

Home health agencies can use their Process Measures Report to monitor their OASIS-based process measures. This agency-level Process Measures Report has three comparison rates:

  • HHA Observed – the current agency performance (not risk-adjusted) for a measure for the selected current period
  • HHA Adjusted Prior – the risk-adjusted agency performance for a measure for the selected prior period
  • National Observed Rate – the average performance of all agencies that have a quality episode of care for the selected period

The Process Tally Report can be used by agencies to look at measure data at the patient-level. The report indicates quality episodes where measures were triggered on the Process Measures Report. These reports can be used to further investigate undesirable process measure data and develop education and/or new policies and procedures to avoid recurrence in the future.
