Not Just Another Report: The 2024 HHVBP Model Annual Performance Report

A home health agency’s (HHA) Calendar Year (CY) 2024 Annual Performance Report (APR) provides the HHA with performance information on a set of quality measures for the CY 2023 performance year. CMS reports an HHA’s adjusted payment percentage (APP), commonly referred to as the payment adjustment, in the HHA’s CY 2024 Annual Performance Report (APR) for each HHA with sufficient data to calculate a Total Performance Score (TPS) for the CY 2023 Performance Year.

An HHA’s APP will be within the range -5% to +5% for the corresponding payment year CY 2025. An HHA’s APP is determined by the HHA’s TPS and the performance of other HHAs in the HHA’s volume-based cohort for the performance year CY 2023.

CMS reports the HHA’s APP in the HHA’s CY 2024 Annual Performance Report (APR) in iQIES. As the percentage by which an HHA’s final claim payment amount under the Home Health Prospective Payment System (HH PPS) will change in accordance with the methodology described in § 484.370, the APP is applied by CMS to an HHA’s payment for each final Medicare fee-for-service (FFS) claim submitted with a payment episode “through date” in the payment year, which is CY 2025.

Public Reporting of HHVBP

The information in your HHA’s CY 2024 APR will be used for public reporting of the expanded HHVBP Model. This is planned to start January 2025 on the CMS Provider Data Catalog (PDC). Each quality measure’s benchmark and achievement threshold by volume-based cohort will be available on the PDC. And for each HHA that qualified for a payment adjustment based on CY 2023 performance, the HHA’s applicable measure results and improvement thresholds, TPS and TPS percentile ranking, and APP will be reported.
