Beginning January 1, 2025, all assessments with a M0090 date of 1/1/2025 or after will use the OASIS-E1 version. Included in this OASIS is the new item for collection, O0350 – Patient’s COVID-19 Vaccination is Up to Date. This item will be collected at the discharge, transfer and death at home time points and is intended to report if the home health patient is up to date with their COVID-19 vaccine status.
In order to determine if a patient is up to date with their COVID-19 vaccination status for this item, providers are directed to the CDC’s webpage: “Stay Up to Date with Vaccines”.
Here providers can access the current definition of “up to date” in order to accurately record the vaccination status of home health patients.
In order to determine the patient’s vaccination status, review the patient’s medical record or documentation of COVID-19 vaccination and/or interview the patient, family or other caregivers or healthcare providers to determine whether the patient is up to date with their COVID-19 vaccine.
If there is conflicting information regarding a patient’s vaccination status, use clinical judgement to determine if the patient is up to date with their COVID-19 vaccines based on information from any available source.
If patient is not up to date because the patient does not meet the CDC’s definition of “up to date”, Code 0, No for O0350. This includes patients who have not received one or more recommended COVID-19 vaccine doses for any reason including medical, religious, or other qualified exemptions. Additionally, this includes patients for whom vaccination status cannot be determined.
If the patient is up to date because they meet the CDC’s current definition of “up to date”, Code 1, Yes for O0350.
Responses from this OASIS item will be used in a process quality measure: COVID-19 Vaccine: Percent of Patients/Residents Who Are Up to Date. This data is also being collected at the end of care episodes or stays across the other post-acute care settings (IRF, SNF, LTCH) as a harmonized cross-setting measure.
This measure calculation will be as follows:
- The number of home health quality episodes in which the patient was determined to be up to date with the COVID-19 vaccination during the reporting period (Numerator),
divided by - The number of home health quality episodes ending with a discharge, transfer to an inpatient facility, or death at home during the reporting period (Denominator).
There are not any exclusions for this process quality measure as this measure’s intent is to report information that can prevent spread of COVID-19 infections. Additional and specific details related to this quality measure can be found here.
As we begin the collection of this new item, O0350, and the calculation of this new measure, home health providers can demonstrate the impact/prevalence of the COVID-19 vaccination in the home health population.
