If your organization prefers to pay via paper check or to register via mail, you may print and complete the form below to order any OASIS or coding books that we stock.
These generic forms can be used to register for a workshop in any location shown on this website (OAI Sponsored workshops only). Registering online is always the preferred method and you may use your credit card to complete an online registration through our shopping cart.
If you do not have a credit card or your organization prefers to pay by paper check, please print out the appropriate workshop form, complete it (including the location where the workshop is held) and return it per the instructions on the form.
The COS-C Exam is offered in two ways:
1) Paper and Pencil Test taken in conjunction with/at the same location as a Blueprint for OASIS Accuracy workshop
2) Computer Based Test – exam candidates can schedule to take the test on a computer at approved testing centers
Register online on our website to apply to sit for the COS-C exam. If you or your organization wishes to pay exam fee by check or otherwise prefers to register by mail, these forms are available to accomplish that.
The forms below are generic and, for the paper and pencil version, you must be certain to include the appropriate location and exam date information. CBT applications take up to 48 hours to process and you will receive email confirmation of your registration directly from the proctoring organization when that process is complete.
You may register for OASIS Answers’ Quarterly OASIS Update Calls via mail, email or fax if you do not wish to utilize our online ordering/shopping cart process. This would allow you to mail a check if you or your organization wished to pay via paper check as electronic checks are not currently accepted on our site. Please print, complete and submit your registration and payment as indicated on the form.
Please remember that you will receive supporting documents to be used on the call along with all teleconference access credentials and document download links via email by close of business (Pacific Time) the day prior to the teleconference.
