CMS Post FINAL CY 2025 APRs in iQIES

CMS has posted the Final Calendar Year (CY 2025) Annual Performance Reports (APRs) in iQIES on November 13, 2025. APRs are only available to HHAs through iQIES.

CMS provides home health agencies (HHAs) with 3 versions of the APRs:

• Preview APRs (just posted)
• Preliminary APRs (posted September 30, 2025)
• Final APRs (typically posted in December)

As a reminder, an HHA receives a CY 2025 APR if the HHA:

• Was Medicare-certified prior to January 1, 2023,
• Had a prior year payment amount and
• Had sufficient data for at least five quality measures to calculate a TPS and APP.

Depending on an agency’s APP, CMS will apply a payment adjustment of a maximum of 5% upward or downward to Medicare’s HH PPS final claim payment amount with a through in CY 2026.

CMS does note that no reconsideration requests – and as a result no requests for Administrator review – were received after the Preliminary CY 2025 APRs were published. As such, the APP presented in the Final CY 2025 APR is the same as the APP reported in the Preliminary CY 2025 APR.
