CMS Posts HHVBP Preview CY 2025 APRs

CMS has posted the Preview Calendar Year (CY 2025) Annual Performance Reports (APRs) in iQIES. APRs are only available to HHAs through iQIES.

CMS provides home health agencies (HHAs) with 3 versions of the APRs:

1. Preview APRs (just posted)
2. Preliminary APRs (posted in September-October)
3. Final APRs (posted in December)

Annual Payment Percentage (APP)
The APP is the specific percentage by which an HHA’s Medicare payment is increased or decreased. It is determined by comparing an agency’s Total Performance Score (TPS) against the scores of its peers in the same volume-based cohort. The adjustment can be upward, downward, or neutral, and can range up to 5% in either direction.

The CY 2025 APRs provide the agency’s Annual Payment Percentage (APP) based on performance in CY 2024, the performance year, which will be applied to Medicare Fee-for-Service (FFS) claims with through dates in the payment year, CY 2026.

In addition, the CY 2025 APRs provide information related to HHA performance on key metrics that feed into the HHA’s APP, including performance measure scores, Improvement Points, Achievement Points, Care Points, and the Total Performance Score (TPS).

An HHA receives a CY 2025 APR if the HHA:

• Was Medicare-certified prior to January 1, 2023, and
• Had sufficient data for at least five quality measures to calculate a TPS and APP.

The following table outlines the performance periods for each measure category included in the APRs.

IMPORTANT NOTES

1. RECALCULATION REQUEST DEADLINE: Once published, HHAs have 15 calendar days to submit a recalculation request if they find evidence of an error in their report. The deadline for submitting a recalculation request for the Preview CY 2025 APRs is September 5, 2025.
2. TPS/APP CHANGES CAN OCCUR: A home health agency’s (HHA’s) Total Performance Score (TPS) and adjusted payment percentage (APP) may change from what they receive in their Preview APR and Preliminary APR, with the final results that will impact the CY 2026 payment year being made available in the Final APRs posted in December.
