CMS Posts Preliminary CY 2025 APRs in iQIES

The Centers for Medicare & Medicaid Services (CMS) has released the Preliminary Calendar Year (CY) 2025 Annual Performance Reports (APRs) for Home Health Value-Based Purchasing (HHVBP) programs in iQIES. These reports are available exclusively to home health agencies (HHAs) through the iQIES system.

CMS provides home health agencies (HHAs) with three versions of the APRs:

1. Preview APRs (just posted)
2. Preliminary APRs (posted September 30, 2025)
3. Final APRs (typically posted in December)

Key Update for Larger-Volume HHAs

It is important to note that because of the recalculation request process, CMS adjusted the CY 2025 APR data used for HHAs in the larger-volume cohort. This adjustment resulted in a slightly higher Adjusted Payment Percentage (APP) for some HHAs in the larger-volume cohort.

CMS has asked agencies in the larger-volume cohort to please compare their HHAs APP in the Preview CY 2025 APR to the APP reported in the Preliminary CY 2025 APR to determine if the agency was impacted. There were no changes made to the smaller-volume cohort APP.

As a reminder, an HHA receives a CY 2025 APR if the HHA:

• Was Medicare-certified prior to January 1, 2023, and
• Had sufficient data for at least five quality measures to calculate a TPS and APP.

The following table outlines the performance periods for each measure category included in the APRs.
