Discharge Function Score Measure: Now on Home Health Review & Correct Reports!

Changes to Review & Correct Reports

The Discharge Function Score Measure is an OASIS-based measure that reflects the percent of home health patients who meet or exceed a risk-adjusted expected function score at discharge. It is unlike other OASIS-based measures, as it is calculated by taking the number of quality episodes from an agency with an observed discharge function score for each OASIS GG0130 – Self-Care and GG0170 – Mobility item that is equal to or higher than the calculated expected discharge function score.

The Discharge Function Score measure is the first home health measure to use an expected score. This expected score is determined by applying a regression equation determined from risk adjustment to each home health stay.

Data related to an agency’s observed discharge function score and the number of home health episodes triggered for the measure are found on a provider’s Review & Correct reports.

As of April 1, 2024, Discharge Function Score measure data with a quarter end date of March 31, 2024 can be found on providers’ iQIES Review & Correct reports. This new measure will be publicly reported for the first time with the January 2025 Care Compare refresh.

In conjunction with this measure addition, data for the Percent of Patients with an Admission and Discharge Functional Assessment and a Care Plan That Addresses Function measure will no longer be displayed on providers’ Review & Correct reports.
