Two HHQRP Quality Measures:

  1. Application of Percent of Residents experiencing One or More Falls with Major Injury
Did you know this process measure has undergone increased scrutiny recently due to the report issued last fall by the Office of the Inspector General? During the presentation we covered important details regarding the measure, how this measure is presented in HHA iQIES reports, OASIS coding guidance for J1800 and J900, and three scenarios to assist you and your staff to think through how to accurately code these OASIS items.
  2. Discharge to Community – Post-Acute Care
Did you know this claims-based outcome measure has been reported on Care Compare since 2019 and will be included in the expanded HHVBP Model measure set beginning CY 2025 impacting CY 2027 payment? To assist your review of your agency’s measure results, we provided essential details regarding this measure, including measure exclusions and definitions of “home health stay” & “community”. Following a visual of how this measure is presented in iQIES, up-to-date CMS measure resources were provided.
