OASIS Conventions Series, Part 1: Assessment Strategies for Accurate OASIS Data Collection

During our Blueprint for OASIS Accuracy workshops, we often engage in rich, meaningful discussions about the diverse methods providers use when completing the OASIS assessment.

These conversations reflect a central truth: It’s simply not possible to account for every clinical scenario within formal guidance.

That’s why Blueprint workshops serve as a valuable opportunity for participants to share real-world experiences and learn from one another.

Our collective focus centers on the OASIS Conventions—the general rules that guide accurate OASIS data collection. By grounding ourselves in these conventions, we can better evaluate the complex social and environmental contexts that arise in practice, applying available guidance, interdisciplinary input, and sound professional judgment.

One important OASIS Convention addresses assessment strategies for completing OASIS items (unless otherwise specified in guidance). These strategies include direct observation, patient or caregiver interviews, collaboration with other agency staff, and record review, among others.

Certain strategies are naturally more appropriate in specific situations. For example, record review can be particularly useful for documenting a history of cognitive issues. However, as often affirmed in our workshop discussions, direct observation remains the gold standard for assessing physiological and functional status.

When providers make full use of these strategies—whenever appropriate and permitted—they maximize both the accuracy and integrity of OASIS data collection. This ensures each patient’s clinical picture is captured with fidelity, in alignment with both best practices and the expectations set forth by the OASIS conventions.
